All of these forms are in Adobe PDF format and you will need the free Adobe Acrobat Reader software (version 4.0 is recommended) to view them. If you do not already have the software, you can download it for free at Adobe Acrobat Reader. There are directions for downloading the software on the Adobe page.

Simply select the form as you need it, print it out and mail it to the appropriate person. You can save any form to your computer if you wish after you have opened it with the Adobe Acrobat Reader.

NYSARC reporting form. The Federation of New York State Bird Clubs established an avian records committee to track "rare birds" seen in NY. The committee - NYSARC - would like written reports of certain species. A list of those species can be obtained here.
